I played the Lord of the Rings RPG where Frodo can straight-up die and the game just keeps on going
The 1990 CRPG The Lord of the Rings, Vol. I by Interplay is a cool, albeit not good, game that introduced forward-thinking mechanics like NPC schedules and permadeath for any character, including Frodo. Despite its innovative ideas, the game and its sequel were not commercially successful and have largely been forgotten.
